Before you register on line please have a scanned birth certificate in JPG format <1 Mb and a picture of players head only in JPG format <1 Mb, that is ready to be uploaded.

Registration is done online only. Please follow steps below:

1. Cleveland Select Registration form REGISTER ONLINE (US Club Soccer)
   Please check the Birth certificate that you will UPLOAD, that is NOT BLURRY
   This registration is mandatory for all Cleveland Select Players
2. Email this Form to danigiulve@gmail.com Sudden Cardiac Arrest and Lindsay's Law form.
3. Venmo Fees to @daniel-giulvezan
4. Please go to shop/merchandise and order your uniform, and any other Club gear that you wish. For the first game you can wear a yellow t-shirt till you get your uniform.
5. CS requires that the player’s soccer ball is INFLATED AT ALL CS activities
6. Players must WEAR the training jerseys at all practices.
